Please submit this completed form online or call 301-853-5363, Monday-Friday, 8:30 a.m.-5:00 p.m., to schedule an appointment.

Fingerprinting is by appointment only. Completing this form will assist us in determining the most convenient time and location for your fingerprints to be taken. You will be contacted within two business days to schedule an appointment. Group appointments cannot be accepted. Please note: posted days and hours may vary depending on demand. Offices may close for holy days and federal holidays.
